Social Services
Case Management/Social Services are provided Monday through Friday from 8:30AM-5PM. The patients served are the medically complex, acutely ill population that requires intensive Case management. The Case Managers and Social Workers act as liaisons between the family, patients, and staff.
The Case Management department facilitates weekly team conferences between family/patient and interdisciplinary team for updates on patient’s clinical status, plan of care, interventions, goals, weekly progress and discharge planning. Also, we provide assistance for families/patients with placement, equipment, support services, referral to community agencies, emotional and psychosocial needs |
