Medical Records
To obtain a copy of your medical record simply print out the Authorization Form, complete and sign the form, then fax it to (310) 768- 2635 or mail the form to us.
*The form should be completed in its entirety. An incomplete or unsigned Authorization Form cannot be processed.
As soon as we receive your request, your request will be processed within 15 Calendar days.
The Health Information Staff will make the arrangement to either mail your record to the address provided, or to schedule a pick up time between 8:00 a.m. to 4:30 p.m.
If your physician or another hospital needs your records for continued care, we will make sure that the records are either faxed or mailed within a reasonable time.
There is a 25¢ per page photocopying fee due before a copy is released for personal access (Health and Safety Code 123100). An invoice may be mailed prior to the actual records being copied, if requested. There is no charge if sent to physicians for health care reasons.
We accept cash, checks or money orders made out to “Vista Hospital of South Bay”. Please do not mail cash payments. We are unable to accept credit card or ATM payments.
Authorization Form

NOTE: Your spouse, partner, child, relative or friend cannot request a copy of your medical records, or pick records up on your behalf, unless they have a power of attorney over you for your healthcare decisions. A copy of the power of attorney for healthcare decisions must accompany the request.
Behavioral Health records are released only upon the professional judgment of the psychiatrist who cared for you while you were here. He/ she may deny you access to your record.
Hospital of Southbay medical records are kept 10 years for adults and 7 years for minors after they reach maturity age.
DEATH CERTIFICATE QUESTIONS
The hospital does not issue Death Certificates.
